Adult
Adult immunisation at Chaslton medical Group focuses on protecting individuals from vaccine-preventable diseases. We offer vaccinations tailored to your age, health status, according to recommended NHS vaccination schedules.
Children
At Chaslton medical Group, we understand the importance of safeguarding the health of your little ones. Our child immunization services ensure that your children receive the recommended vaccines at the right age to protect them from various diseases, promoting their well-being.
Flu
Each year, we provide the seasonal flu vaccine to eligible adults and children to reduce the risk of flu-related illnesses. It’s a crucial step in safeguarding your health during flu season, especially for vulnerable individuals.
COVID
Chaslton medical Group is actively involved in the national COVID-19 vaccination program. We offer COVID-19 vaccines to eligible individuals following government guidelines, contributing to the effort to control the spread of the virus and protect our community.
Contraception
For contraception appointments, you will be scheduled with our nursing team. Simply inform the reception staff of the reason for your call. We can provide the contraceptive pill or injection.

Asthma /COPD
Where possible we try to get as much information about your asthma control before your review. This might include a text or postal questionnaire. If your asthma is well controlled we will aim to do the review remotely. If we feel your asthma needs review you will be invited for a face to face appointment with one of our asthma nurses or GPs.
Diabetes
All people with Diabetes should have annual and 6 monthly reviews.
For your annual review you will first receive an appointment with one of our nursing team. This will include blood tests, blood pressure and a foot check (unless you are under podiatry). You will then have a follow up appointment with one of our diabetic team to discuss your results.
The 6 month review involves bloods and blood pressure monitoring. Your results will be reviewed by our in house diabetes team. If everything is satisfactory you should receive a letter to inform you of your results. If we need to talk to you about your results, You will be invited for an appointment to speak with one of the team
Long-term Conditions
These are conditions that you live with a long term basis. A number of these conditions require regular review to make sure they are well controlled. Examples of long term conditions include hypertension, heart disease, diabetes and asthma.
If you have a “Long Term Condition” we will invite you in around your birthday month for a review. We aim to review all of your long term conditions at the same time. This might include appointments with our nursing team, GPs and pharmacists. Sometimes you will need a face to face appointment after your initial checks. If a face to face appointment is not needed, you should receive a letter updating you on your results. We aim to have this with you within 28 days of your review, but in busier times this may be longer. We strongly encourage you to participate in this review as it allows us to ensure your condition is well controlled and your medication reviewed appropriately.
Cervical Smears
You will be sent a reminder when your smear test is next due, but if in doubt, please ask at reception. Appointments for smears should be made with the practice nurse.
Baby and Postpartum Service
Babies are invited for an 8 week baby check. This is similar to the new-born check, aiming to detect any problems with eyes, heart or hips promptly, so that appropriate action can be taken. Mothers are also offered a postnatal check around 8weeks. This is a separate appointment to focus on issues pertinent to the time shortly after your baby has been born, for example reviewing your blood pressure, mood and contraceptive choices. If you have not heard about your appointment and you are approaching 8 weeks since delivery, please contact the surgery so we can arrange this for you.
Well Baby and Child Surveillance
A weekly well baby and child surveillance clinic is held at Chastleton by appointment. A doctor and practice nurse are in attendance.
