Important Update: Total Triage Begins on 30th June 2025
From 30th June 2025, we are excited to announce that Chastleton Medical Group will move to a Total Triage system.
Total Triage means that every request for a GP and nurse practitioner appointment will be assessed by a duty Doctor (GP) before an appointment is booked. This helps us direct you to the right care quickly and fairly.
Please note this does not include nurse appointments please ring as normal for these.
How Total Triage works with Accurx (A brief introduction)
With the launch of Total Triage this means from 2 weeks before 30th June there will be no pre bookable appointments available.
What is Total Triage and why are we Changing?
Total Triage allows clinicians to assess patients’ request promptly and ensures patients see the right healthcare professional at the right time. We want to ensure appointments are allocated based on clinical need. Total Triage helps us organise requests so patients who need urgent same-day care can be prioritised while also providing timely care for routine issues.
You may be directed to anyone of the multidisciplinary team best suited to care for your needs. This could include:
- Pharmacy First
- First contact mental health practitioner
- Physiotherapist
- Pharmacist
- Nurse Practitioner
- Ophthalmologist
- Podiatrist
- GPs
- Social prescriber

How Total Triage will work
You will submit a request for an appointment, ideally this should be done online but for those who are unable our reception team will manage this via the phone.
The duty Doctor will triage this request
You will be informed of the outcome via text or telephone.
Please note all GP requests will be accessed via this system. People who present to the desk will be asked to complete the same form.

Over the phone
For those not able to access online forms, our reception staff will fill out the same form for you over the phone. There is no difference in how your form is processed, so you receive the same care regardless of how the form is submitted.

Possible Outcomes
- Same-Day Appointment.
- Routine Appointment (within 5 to 10 days).
- Advice (e.g., self-care recommendations, signposting to community pharmacy).
- Referral to other services or professionals (physiotherapists, mental health practitioners, etc.).
If a clinician decides you need a face-to-face appointment, they’ll send you a booking link via text or call you. Simply click the link and choose from the available slots. If you’d prefer to book by phone or can’t use the link, call reception and they’ll assist you.
