Appointments
Appointments can either be booked up to two weeks in advance (non-urgent or review appointments) or booked on the day (acute or urgent problems).

Out of Hours
If you require urgent attention or advice, telephone 111. If possible, have a pen and paper handy. Out of hours provision is provided by County Durham PCT.

Test Results
Please could you telephone the surgery for your results on: 01913846171 between 2.00- 4.00 pm Monday – Friday

Cancelling your Appointment
If you are unable to attend an appointment with one of the doctors or nurses, please telephone or use the link at the bottom of this page to cancel your appointment.

New Patient Questionnaire

The practice boundary was redefined on the 1st of April 2004. The new boundary excludes Gilesgate Moor, Gilesgate, Carrville, Belmont, High Shincliffe, Shincliffe, Langley Moor and Broompark. Patients residing in these areas will not be affected by these changes unless they move to another address, which is also outside the boundary.

To register as a new patient with the practice patients must reside within the defined practice boundary which include the following areas:

Durham City
Earls House
Farewell Hall
Finchale Priory
Framwellgate Moor
Nevilles Cross
Newton Hall
Merryoaks
Pity Me
Sniperley
Whitesmocks.

Registration with the practice

If you wish to register with the practice please attend the surgery with your medical card (if available) or you can complete a registration form at the practice. Alternatively you may print off a registration form, (using the link below), fill it out and bring it in with you on your first visit to the practice.

As a new registration to this practice, we are obliged by the NHS Counter Fraud Service to assess your eligibility to receive free NHS treatment. Entitlement is based on legal residency, irrespective of nationality, citizenship or any previous payment of NIC's or taxes. Please bring with you proof of ID and residency. A combination of the following can be accepted as identification (it is preferable that one item of photo ID is seen):

Birth certificate, Marriage Certificate, Medical Card, Driving Licence, Passport, Local Authority Rent Card, Paid Utility Bills, Bank card/statements, P45, N.I. Number ard, Payslip, Papers from Home Office, Letter from Benefits Agency/benefit book/ signing on card.

The following documents will not be accepted:
Library card, video rental card, health club card, private rent book.

Suspected false registrations will be reported to Counter Fraud Specialist.
